The Wyoming Downs OTB Incident Report fulfillment process will show four (4) various stages of the Incident Report as it is goes through the process of being reported, reviewed, approved and closed.
1st: OPEN
When an incident is first reported and submitted using the Incident Report Portal, the Incident Report Status is OPEN. At this stage, the incident data entered, collected, and relevant attachments are just reported and submitted. The Incident Report is visible to the author/reporter of the Incident at the Incident Report Portal and can be to changed and commented by the author/reporter of the incident when logged in.
Transition: The Risk Manager receives the incident report, reads and determines next steps. At this point, the Risk Manager can route the Incident for Review or for Approval.
2nd: IN REVIEW
The Risk Manager makes a determination if the Incident requires further review because of missing or incomplete information. Incident Report is then routed for review using the Agent's Portal. The OTB Site Manager (contact of the Incident Report) will be notified through email and will need to log into the
Incident Report Portal to make the necessary changes or to provide the requested information. The OTB Site Manager makes a comment on the Incident Report to notify the Risk Manager for the changes and/or additions to the Incident Report.
Transition: The Risk Manager receives notification (through EMail and Agent's Portal) once the requested changes and/or additions are completed, and makes the determination to send for further review to the OTB Site Manager or send for Approval.
3rd: FOR APPROVAL
The Incident Report is thoroughly reviewed and determined by the Risk Manager as completed with all the required information. The Risk Manager submits and routes the Incident Report for Approval. The approver/s of the Incident Report will be notified to approved the Incident Report using the Agent's Portal.
4th: CLOSED
The final status of the Incident Report is CLOSED. Once the Incident Report is approved, the incident can be closed by the Risk Manager using the Agent's Portal.
At each stage of the Incident Report, certain notes, explanations, descriptions and/or instructions can be placed/entered in the comment section of the Incident Report. Also, relevant documents can be attached to the Incident Report.
